The narrative follows Shimizu Hinako, a seemingly ordinary teenager whose life takes a dark turn when her town is engulfed in fog, transforming into an unrecognizable nightmare. As Hinako, players will navigate this transformed town, solve puzzles, combat bizarre adversaries, and make critical decisions to survive. The story promises a journey culminating in a "beautiful yet terrifying choice."

Series producer Motoi Okamoto highlighted the core concept of Silent Hill f: "finding the beauty in terror." Okamoto emphasized that players will experience this world through the eyes of a young girl confronted with a hauntingly beautiful decision.

Okamoto confirmed that Silent Hill f offers a standalone narrative, welcoming to newcomers while still rewarding long-time fans with hidden Easter eggs. As a fan of the franchise, Ryukishi07 aims to honor Silent Hill's roots while pushing the series forward, challenging the notion of what constitutes a Silent Hill game outside its traditional setting.

Ryukishi07 expressed confidence in their creation, eagerly anticipating feedback from the series' dedicated fanbase. He remarked, "Naturally, from a creator’s standpoint, I feel I can confidently say that what we’ve made is a Silent Hill game. However, we’re interested in seeing how long-time fans of the series feel after playing, and if they agree."
